YOUNG ADULTS - DO YOU HAVE A CARE MANAGER OR SOCIAL WORKER?

If you would like to be referred to the Fairway project and you do not have a social worker please:

• Speak to your guidance teacher at school or college

• Contact your social work department’s Enquiry & Information team, or sometimes called Reception services and ask for a social work referral and an assessment of need

If you already have a social worker or a care manager:

• Contact your social worker and ask if you can be referred to the Fairway Service

• And if you receive Direct Payments or Independent Living Fund (ILF), contact Fairway and ask to purchase the service

Your local Enquiry & Information team or Reception service is your first point of contact for information, advice, and an initial assessment of your care needs as well as in emergency situations. They will also offer benefit checks, to ensure that you are getting all the benefits you are entitled to.

Your Fairway facilitator will help you to make this contact and will also help you if you need further support when communicating with social work.

Your social worker will carry out an assessment of your needs. This means what kind of support you will need to be able to do the things you want to do e.g. access college, shopping, budgeting your household bills, meeting up with your friends, being safe etc..
